When you complete a Prescribe Go! check-in, we may collect:

- Postal code area (first three characters only)
- Responses to wellbeing questionnaires
- Responses to mood screening questions
- Program participation preferences
- Attendance information
- Voluntary comments or information you choose to provide
Provider Account Information
For provider and administrator accounts, we may collect:
- Name
- Organization name
- Email address
- Telephone number
- Postal code area
- Login credentials
Passwords are stored in encrypted form and are never stored in plain text.
Why We Collect Information
We collect information to:
- Match participants with appropriate community activities and services
- Facilitate referrals and participation
- Measure program engagement
- Evaluate outcomes and effectiveness

Mood and Wellbeing Information

Providing this information is optional; however, certain recommendations may not be available without it.
